When outcomes arise through multiple agents, roles, procedures, technologies, and institutions, responsibility may become differentiated across several organisational locations. Complexity does not make responsibility vanish. Moral Evaluation must consider who acted, who omitted, who designed, who authorised, who benefited, who could intervene, and which structures helped sustain the resulting consequences.
